Before submitting a virtual EMS access form, please obtain departmental approval to be able to request or reserve SPH space on behalf of your unit.

  • Please provide a UIC C-FOAPAL account number. This information is needed for billing purposes and will be added to the user account.

  • Upon completion, an SPH EMS administrator will reach out to provide virtual EMS user account credentials. Please allow at least 24 hours for account creation to be processed and completed.
