Please note the documents in steps 2 and 3 will be uploaded via the online application system. For letters of recommendation, you will upload the names and contact information of references. References will then be automatically contacted to submit their recommendation.
- Complete the UIC graduate application and fee: UIC requires an application and non-refundable application fee ($70 for Domestic and International applicants). Employees of UIC may be eligible for a fee waiver. A mandatory, non-refundable $100 International Credential Evaluation Fee is assessed for all graduate and professional degree-seeking applicants who have post-secondary coursework taken outside the United States.
- Personal Statement. It is generally recommended to keep the statement to 500-800 words and to focus on your interests/experiences in public health and the program you are applying to specifically, your goals with the degree, and why you would be successful in the program.
- Resume or CV.
- Three letters of recommendation.
- Official Transcripts. Official transcripts are required only for the highest degree earned. The requested official transcripts must be uploaded to the UIC Application; UIC Admissions will not accept postal or emailed transcripts.
Please note: A graduate level course in biostatistics is a requirement of admission. Matriculants who lack this prerequisite course will be required to complete a graduate level biostatistics course during their first year in the program. Credits earned to complete the biostatistics deficiency will not be counted toward the 48 credit hour minimum to complete the degree.
