Admission Requirements

Application to the Master of Public Health in Public Health Informatics program must be completed with the submission of two forms: the Schools of Public Health Application Service (SOPHAS) form and a UIC supplemental application fee form. Please note that there is a fee associated with each form.

Applicants must also submit the following supplemental documents:

  1. Transcripts
  2. GRE Scores
  3. Curriculum Vitae
  4. Statement of Purpose and Objectives

  1. U.S. TRANSCRIPTS - Applicants must provide a separate official transcript from every US institution attended. All official transcripts must be sent directly to SOPHAS. While a grade point average (GPA) of 2.75 is required, a GPA close to 3.0 (on a 4.0 scale) is recommended. See below for information about foreign transcripts.
  1. GRE SCORES- Combined Graduate Record Examination (GRE) scores of at least 1000 (verbal plus quantitative) are required. Official, certified or attested copies of the GRE test scores must be sent directly to:

  2. CURRENT RESUME OR CURRICULUM VITAE - Upload in the CV/resume section of the SOPHAS application.

  3. STATEMENT OF PURPOSE AND OBJECTIVES - Provide a personal essay in SOPHAS describing your past education, experience and current professional career objectives. You are encouraged to comment on any or all of the following: plans you have to use your education and training; the needs and/or challenges you perceive as important in your field of study; and any personal qualities, characteristics and skills you believe will enable you to be successful in your chosen field of study. Applicants to the MPH PHI program who are interested in applying to the Peace Corps Master’s International Program should indicate their interest in the statement.

  4. REFERENCES - Provide 3 letters of recommendation from persons qualified to assess your academic work; clinical, public health or professional experiences; or leadership potential in public health. Letters of recommendation may be sent directly to SOPHAS in online or in paper format. NOTE: Electronic submission is preferred.

Additional Requirements for International Applicants

  1. International academic documents that are evaluated by a professional credential evaluation service that holds membership in the National Association of Credential Evaluation Services (NACES) are also acceptable.

  2. TOEFL SCORES - International applicants must present evidence of English competency. Official scores from the Test of English as a Foreign Language (TOEFL) administered by the Educational Testing Service, Princeton, NJ 08540, must be sent to UIC. UIC's institution code for the TOEFL is 1851. The test must have been taken within the last two years. Minimum scores required: paper-based TOEFL = 550, computer based TOEFL = 213, IBT total score = 80. The USMLE and ECFMG will not be considered in lieu of the TOEFL or as evidence of English competency. See the FAQ page of our Web site for information about waiving the TOEFL requirement. TOEFL scores may be submitted directly to Code 5688 SOPHAS in Massachusetts.

Application Instructions

SOPHAS application

  1. There are 15 sections which make up the SOPHAS application. To begin applying, please visit www.sophas.org. View the SOPHAS abbreviated instructions document prior to beginning your application.
  2. We also recommend viewing the detailed instructions available on UIC's School of Public Health Admissions Web page.

    NOTE: In section 15 of the SOPHAS application form applicants must designate the schools and programs for which they are applying. The following information should be entered in the Designations section to apply for the UIC Master of Public Health in Public Health Informatics.
    • School Designations – Select “University of Illinois at Chicago School of Public Health”
    • Degree - Program / Department, select "MPH - Public Health Informatics"

UIC School of Public Health supplemental application

  1. UIC requires a non-refundable supplemental application fee made payable to the University of Illinois by check, bank draft or money order only ($60 for both domestic and international applicants). For further instructions on how and where to submit your payment, please visit http://www.uic.edu/depts/oar/grad/payment_grad.html.

  2. Please write MPH Public Health Informatics in the intended program section of the UIC supplemental application fee form.

  3. To help us track your application package accurately, we request that you send an e-mail to the academic program coordinator, stating that you have initiated application to the program.

 

Tuition and Fees

Tuition and fees for the Master of Public Health in Public Health Informatics program is currently $793 per credit hour. NOTE: Tuition and fees are subject to change without notice any time prior to the first day of instruction.
