Making a Reservation
SPH ONLINE RESERVATION REQUEST SYSTEM
Before you begin
Be sure to observe the regulations applicable to use of the SPHPI facilities before making a reservation.
In order to make a request for space, you must have an account in the Online Reservation System. Please contact Ms. Tonga Alexander at (312) 996-6620, or via email at email@example.com, and provide her with your name, title, organization, mailing address, phone number and email. She will create an account for you and email your password to you.
Once you have your account, you can use the Online Room Reservation System to search for available space and create a reservation request for your event. You may also request audio/visual equipment and services at that time. Please refer to the Quick Reservation Guide for more information.
Upon submission, you will receive an email summary of the request. It is ONLY A REQUEST at this stage. The request will be sent to SPH staff, who will review it, reserve the space as “Tentative” and begin development of the contract.

Ms. Alexander will finalize the contract and forward it to the event contact person. Return the signed contract and your deposit or payment to Ms. Tonga Alexander at the UIC School of Public Health, 1603 W. Taylor St. , Chicago , IL 60612
Upon receipt of the payment, Ms. Alexander will confirm the reservation and all services.
Changes to your Reservation
PLEASE NOTE: Changes to event information cannot be made online once the status has changed from “Web Request” to “Tentative” or “Confirmed.” Please contact Ms. Alexander with your changes either by telephone (312) 996-6620, fax (312) 996-1374, or email email@example.com.
Prior to Your Event
Contact Mr. Jose Alvarez at (312) 413-2012 or by email at firstname.lastname@example.org about posting informational and/or directional signs. All signs must be posted with non-acid tape. See regulations applicable to use of the SPHPI regarding signs (item 8).
Users are responsible for any and all catering needs and services. While SPHPI staff are not involved in selection or provision of services, we have offered information we've gathered on local catering services. Be sure clear delivery instructions are specified on the invoice for the driver and to have staff on site to receive the delivery.
Day Of Event
Ensure that clear signage to direct guests to proper room(s) has been posted.
Paulina parking garage is available across the street at Taylor and Marshfield (enter on Marshfield or Paulina). Street parking is available at metered spots.
Clean up and return all supplies. If space is not left in agreed upon condition, you will be charged custodial service. You will also be charged for any equipment or supplies not returned at the agreed upon time.
